Accident resulting in personal injury or equipment damage<br />

Delay of more than 1 hour on vessel discharge due to: Customs, USDA, loss of crane<br />

or critical materiel handling equipment support, weather<br />

Labor non-availability<br />

Dead time exceeding 30 minutes<br />

Train departure delayed more than 1 hour<br />

Truck driver waits more than 1 hour for CBL (goal is less than 15 min)<br />

IT Systems down (WPS/EEDSK/GFM/ETA/Powertrack)<br />

Commercial activity adversely affecting vessel schedule<br />

Final destination change<br />

Misrouting of any equipment<br />

Visitor (lieutenant colonel or equivalent and above)<br />

Priority Information Requirements<br />

(Report affirmative answers to commander, 842d, upon discovery.)<br />

Will any vessel be delayed in its scheduled arrival due to weather, maintenance, or other<br />

reasons, with latest time information of value to known berths?<br />

Are there any commercial vessels whose berthing schedules might conflict with a vessel<br />

carrying military cargo?<br />

Is there any civilian cargo staged on the port which will conflict with our current staging<br />

plan, or cause it to change?<br />

Is there a named tropical storm of any magnitude in Gulf of Mexico?<br />

Are there weather conditions that are likely to impede operations?<br />

Is there a labor dispute of any kind that is likely to delay or suspend operations?<br />

Are there any suspicious persons on the port, the presence of whom could constitute a<br />

breach of security?<br />
