Senator SMITH. Dr. Mans<strong>on</strong>, I w<strong>on</strong>der if you would agree with<br />
the c<strong>on</strong>clusi<strong>on</strong> of some <strong>on</strong> the first panel that "bioidentical" is a<br />
marketing term and it has no medical definiti<strong>on</strong>?<br />
Dr. MANSON. I would agree. I think that there is a great difference<br />
between the way the term "bioidentical" is used by scientists<br />
and the way it is being used by alternative medicine practiti<strong>on</strong>ers<br />
and in the mass media.<br />
The scientists use it for horm<strong>on</strong>es that are chemically identical<br />
to those produced naturally by the body.<br />
There are three types of natural estrogen that women make. In<br />
additi<strong>on</strong>, there is progester<strong>on</strong>e, as well as testoster<strong>on</strong>e, and other<br />
horm<strong>on</strong>es. Many of these horm<strong>on</strong>es, as we have discussed, these<br />
bioidentical horm<strong>on</strong>es, are available through FDA-regulated medicati<strong>on</strong>s<br />
that are produced en masse and available in retail pharmacies.<br />
These custom-compounded horm<strong>on</strong>es, often we d<strong>on</strong>'t even know<br />
what is in them. They do not have any clear advantage over the<br />
bioidentical horm<strong>on</strong>es that include the estradiol or progester<strong>on</strong>e<br />
that are available through a retail pharmacy.<br />
Senator SMITH. Do you know of any head-to-head studies between<br />
traditi<strong>on</strong>al horm<strong>on</strong>e therapy versus bioidentical horm<strong>on</strong>e<br />
therapy?<br />
Dr. MANSON. That is an interesting questi<strong>on</strong>. The <strong>on</strong>ly current<br />
trial is the Kr<strong>on</strong>os Early Estrogen Preventi<strong>on</strong> Study, and it is <strong>on</strong>going.<br />
The results are not yet available. It is a head-to-head comparis<strong>on</strong><br />
of oral c<strong>on</strong>jugated equine estrogens, which were tested in the<br />
Women's Health Initiative, but a lower dose is being tested in the<br />
Kr<strong>on</strong>os trial, and a transdermal estradiol patch.<br />
Senator SMITH. Who is doing that test?<br />
Dr. MANSON. It is being d<strong>on</strong>e by the Kr<strong>on</strong>os L<strong>on</strong>gevity Research<br />
Institute, a private foundati<strong>on</strong>. It is not a drug company-sp<strong>on</strong>sored<br />
trial.<br />
Senator SMITH. Do you think the Federal Government ought to<br />
take the lead in it, or participate in it, or-<br />
Dr. MANSON. I think it would be helpful for the Federal Government<br />
to get involved in providing some support so that women can<br />
get answers to these questi<strong>on</strong>s. So it will be comparing the oral<br />
c<strong>on</strong>jugated estrogens in low dose with the transdermal bioidentical<br />
form of estradiol.<br />
Senator SMITH. You spoke in your testim<strong>on</strong>y about the role of the<br />
physician in prescribing bioidentical horm<strong>on</strong>es. Do they have<br />
enough informati<strong>on</strong> to prescribe them? Are they doing that?<br />
Dr. MANSON. Yes. Some of them are.<br />
I do not think that, with how busy physicians are these days and<br />
all of the other issues that they have to attend to, that most have<br />
really gotten the informati<strong>on</strong> that they need about what bioidentical<br />
horm<strong>on</strong>es are, what custom-compounded horm<strong>on</strong>es are-all of<br />
these issues and c<strong>on</strong>cerns that we have been discussing this morning-and<br />
that they really have a full understanding of what they<br />
are prescribing for their patients because of just a lack of available<br />
informati<strong>on</strong>.<br />
Senator SMITH. I mean, the obvious c<strong>on</strong>clusi<strong>on</strong> is some of them<br />
may unwittingly be practicing some form of quackery by getting<br />
into this area.
