We ask commenters to provide <strong>document</strong>ation to us as to<br />

the type and cost of equipment that is used in furnishing<br />

these services in the physician office and other<br />

information to support any suggested changes from the prior<br />

inputs.<br />

2. CPT Code 41530<br />

The AMA RUC recommended PE direct inputs for CPT code<br />

41530, Submucosal ablation of the tongue base,<br />

radiofrequency, one or more sites, per session, include a<br />

disposable pulse oximeter finger probe. We did not accept<br />

the addition of this item to the PE database for this or<br />

any other procedure because, as we have discussed in the<br />

CY 2004 PFS proposed and final rules (68 FR 49037 and<br />

63206), we continue to treat the pulse oximeter probe as<br />

reusable.<br />

3. CPT Code 46930<br />

In the AMA RUC PE recommendations for CPT code 46930,<br />

Destruction of internal hemorrhoid(s) by thermal energy<br />

(eg, infrared coagulation, cautery, radiofrquency), the<br />

specialty society requested that we add a light guide to<br />

the existing price of $3,087.50, for the infrared<br />

coagulator and provided the list price of $730 for this<br />
