§424.36(b)(6)(ii)(C)(2), as finalized in the CY 2008 PFS<br />

final rule. The commenter also stated that hospitals are<br />

moving toward electronic recordkeeping, and urged us to<br />

clarify that secondary forms of <strong>document</strong>ation used to<br />

verify transport do not need to be signed by a<br />

representative of the facility, provided that the form of<br />

<strong>document</strong>ation obtained is an official facility record that<br />

clearly indicates the name of the patient, and the date and<br />

time the patient was received by or transported from that<br />

facility.<br />

Response: We acknowledge that, although the preamble<br />

language in the CY 2008 PFS final rule stated that all<br />

forms of secondary <strong>document</strong>ation used to verify transport<br />

need to be signed by a representative of the receiving<br />

facility, the regulation text at §424.36(b)(6), as<br />

published in the 2008 CY PFS final rule, did not include<br />

this specific requirement. We are clarifying<br />

§424.36(b)(6)(ii)(C)(2)to provide that secondary forms of<br />

<strong>document</strong>ation used to verify transport do not need to be<br />

signed by a representative of the receiving facility if the<br />

form of <strong>document</strong>ation obtained is an official hospital or<br />

facility record, (such as the facility or hospital<br />

registration/admissions sheet, patient medical record,<br />

facility or hospital log, or other facility or hospital<br />
