providers and suppliers to maintain ordering and referring<br />

<strong>document</strong>ation, including the NPI, received from a physician<br />

or eligible NPP. Physicians and NPPs are required to<br />

maintain written ordering and referring <strong>document</strong>ation for<br />

7 years from the date of service. In addition, we are<br />

finalizing the provision found at §424.535(a)(10) that<br />

states that failure to comply with the <strong>document</strong>ation<br />

requirements specified in §424.516(f) is a reason for<br />

revocation.<br />

Finally, the aforementioned provisions regarding<br />

ordering and referring <strong>document</strong>ation are effective with<br />

services furnished on or after the implementation date of<br />

this final rule.<br />

6. Revocation of Enrollment and Billing Privileges in the<br />

Medicare Program (§424.535(h))<br />

Historically, we have allowed providers and suppliers<br />

whose Medicare billing numbers have <strong>been</strong> revoked to<br />

continue billing for services furnished prior to revocation<br />

for up to 27 months after the effective date of the<br />

revocation. Since we believe this extensive billing period<br />

poses significant risk to the Medicare program, we proposed<br />

to limit the claims submission timeframe after revocation.<br />

In §424.535(g) (redesignated as §424.535(h)), we proposed<br />

that revoked physician and NPP organizations and individual<br />
