Population-based data about the relationship<br />
between CIMT and prehypertension are rare.<br />
Methods: CIMT was measured via B-mode sonography<br />
at plaque free areas 1cm proximal to the<br />
bulb. Systolic (SBP) and diastolic blood pressure<br />
(DBP) were defined according the Seventh<br />
Report of the Joint National Committee<br />
on Prevention, Detection, Evaluation, and<br />
Treatment of High Blood Pressure (JNC-7) and<br />
divided in normotension (SBP < 120mmHg and<br />
DBP < 80mmHg) and prehypertension (SBP<br />
120-139mmHg and/or DBP 80-89mmHg). For<br />
2219 normo- or prehypertensive participants<br />
of the Heinz Nixdorf Recall study (42.8% men)<br />
without coronary heart diasease between 45<br />
and 75 years, data of ultrasound examination<br />
and prehypertension were available. Data are<br />
presented as mean ± SD or estimate (95%CI).<br />
Results: We observed 911 normotensive (27.8%<br />
men) and 1308 prehypertensive (53.2% men)<br />
subjects. Mean-CIMT was significantly higher<br />
in prehypertensives than in normotensives<br />
(0.671mm ± 0.124mm vs. 0.627mm ± 0.110mm,<br />
p< 0.0001). Also in subjects with no antihypertensive<br />
medication (n=705 normotensives,<br />
n=918 prehypertensives), CIMT was<br />
significantly higher in prehypertensives: 0.662<br />
± 0.122 vs. 0.615 ± 0.105, p< 0.0001. A general<br />
linear model for CIMT depending on prehypervs<br />
normotension upon adjustment for age and<br />
gender yielded an estimate of +0.017mm (95%CI<br />
0.007-0.027, p=0.0005) in the full population, and<br />
+0.021mm (0.010-0.033, p=0.0002) for participants<br />
without antihypertensive medication.<br />
Conclusion: Already prehypertensive subjects<br />
showed augmented signs of carotid subclinical<br />
atherosclerosis. Further population-based data<br />
are necessary to support CIMT-measurements<br />
in its role as a supervision examination in prehypertensive<br />
subjects.<br />

Fragestellung:<br />
Die nicht-invasive Abschätzung des zentralen<br />
aortalen Blutdrucks erfolgt (z.B.) auf der Basis<br />
von peripher gemessenen Blutdruckwerten<br />
und der Messung einer peripheren Pulswelle<br />
über eine Transferfunktion. Bislang gibt es<br />
kein international anerkanntes Validierungsprotokoll<br />
von Pulswellenanalysegeräte. Auch<br />
besteht keine Einigkeit darüber, ob invasiv oder<br />
non-invasiv validiert werden sollte.<br />
Methoden:<br />
Wir verglichen in Anlehnung an <strong>das</strong> Validierungsprotokoll<br />
der ESH für Blutdruckmessgeräte<br />
zwei Geräte: Das Referenzgerät<br />
(SphygmoCor®=B) misst tonometrisch und<br />
ist invasiv validiert. Das Vergleichsgerät zeichnet<br />
mit einer Oberarmmanschette oszillographisch<br />
die Druckkurve auf (StabiloGraph<br />
®=A). Pro Patient erfolgten vier Untersuchungen<br />
in randomisierter Gerätereihenfolge,<br />
der Eingangs bestimmte periphere Blutdruck<br />
wurde mit A gemessen. Die statistische Analyse<br />
erfolgte mit Zwei-Faktor-Messmodellen.<br />
Ergebnisse:<br />
Von 90 untersuchten Patienten waren 58 (20<br />
Frauen) komplett auswertbar - 32 wiesen aus<br />
technischen Gründen nicht verwertbare Messungen<br />
auf (A: n=16; B: n=17). Das mittlere Alter<br />
betrug 55,45 Jahre. Folgende zentrale Drucke<br />
wurden gemessen:<br />

Schlussfolgerungen:<br />
• Statistisch findet <strong>sich</strong> kein Unterschied<br />
zwischen den untersuchten Geräten.<br />
• Gerät B weist im Vergleich zu A eine<br />
bessere Reproduzierbarkeit des zSBD auf.<br />
• Für den zSBD ergibt <strong>das</strong> Gerät B bessere<br />
jedoch nicht signifikante Reliabilitäten<br />
•<br />
(Zuverlässigkeit).<br />
Die Fehlervarianz für den zSBD nimmt bei<br />
der Zweitmessung mit dem Gerät A zu,<br />
nicht jedoch mit B.<br />
• Die Betrachtung der Vergleichsgeräte ohne<br />
Berück<strong>sich</strong>tigung der Reihenfolge ihres<br />
Einsatzes zeigt, <strong>das</strong>s Gerät B die deutlich<br />
reliablere Methode ist.<br />
• Bei Anwendung des ESH-Protokolls zur<br />
Validierung von Blutdruckgeräten ergibt<br />
<strong>sich</strong> keine statistisch signifikante Abhängigkeit<br />
der geschätzten Reproduktionsvarianz<br />
von der Höhe des peripheren Blutdrucks.<br />
• Bei Betrachtung eines stetigen Zusammenhangs<br />
zeigt <strong>sich</strong> ein signifikanter Zusammenhang<br />
des zentralen systolischen Blutdrucks<br />
mit dem peripheren systolischen<br />

Background and aim: Studies confirm that an<br />
optimized blood pressure (BP) adjustment, compared<br />
to a standard attended control-group, can<br />
be achieved with a 3-month interventional telemetric<br />
blood pressure measurement (iTBPM) in<br />
inadequately treated patients with arterial hypertension.<br />
This recent analysis should clarify if<br />
these effects lead to a better control in long term.<br />
Methods: 57 patients with initially inadequately<br />
treated arterial hypertension were evaluated in<br />
the former intervention study concerning BP<br />
adjustment after 3 month of iTBPM (besides<br />
standard supply the iTBPM group was supported<br />
additively by a telemetry supply for the daily<br />
transfer of the measured BP values at home<br />
to the physician. In case of an insufficient BPcontrol<br />
or non-compliance, the physician<br />
was able to counteract for example in form of<br />
changing medication or intervention via phone).<br />
In conclusion an ABPM could be carried out<br />
with 18 (iTBPM) and 22 patients (controls) after<br />
a period of 617 and 576 days on average. Those<br />
patients were compared concerning ABPMtarget<br />
values with the terminal data after the<br />
initial 3-month BP adjustment period.<br />
