Schlechte Compliance oder schwere körperliche<br />
Tätigkeit auf Seiten des Patienten sollten<br />
den Chirurgen eher davon abhalten, eine<br />
Prothese zu implantieren. Auch der Wille des<br />
Patienten, weiterhin an Schlag- <strong>und</strong> Rasanzsportarten<br />
teilzunehmen, macht eine strenge<br />
Überprüfung der Indikation erforderlich, da<br />
derartige Belastungen eine frühe Lockerung<br />
<strong>und</strong> PE-Abrieb mit sich bringen.<br />
Zusammenfassung<br />
Der endoprothetische Gelenkersatz des Ellenbogengelenks<br />
hat in den letzten 15 Jahren<br />
eine deutliche Verbesserung erfahren, sowohl<br />
im Hinblick auf die Technik als auch auf die<br />
Implantate. Hierdurch kann nunmehr eine<br />
Vielzahl von Erkrankungen mit guten Ergebnissen<br />
behandelt werden. War die Ellenbogenprothetik<br />
historisch auf den Einsatz im<br />
rheumatischen Ellenbogen beschränkt, so ist<br />
die Indikation zur Implantation um einige<br />
Diagnosen erweitert worden.<br />
Trotz der in der Literatur berichteten guten<br />
Ergebnisse ist die Indikation streng zu stellen.<br />
Aufgr<strong>und</strong> der möglichen Implantatkomplikationen<br />
mit Lockerungen <strong>und</strong> PE-Verbrauch ist<br />
die Ellenbogenprothetik dem älteren Patienten<br />
von über 65 Jahren vorbehalten. Beim jüngeren<br />
Patienten muss neben der zu Gr<strong>und</strong>e<br />
liegenden Diagnose auch seine Compliance<br />
<strong>und</strong> sein Leistungsanspruch in die Entscheidung<br />
mit einbezogen werden. Alternative<br />
Verfahren wie arthroskopische oder offene<br />
Arthrolysen, die ulnohumerale Arthroplastik<br />
(Kashiwagi) oder die Interpositions- oder<br />
Distraktionsarthroplastik müssen mit ihren<br />
Indikationen <strong>und</strong> technischen Möglichkeiten<br />
bekannt sein <strong>und</strong> bei der Differentialtherapie<br />
mitbedacht werden.<br />
