The Therapeutic History of a Recalcitrant Case of Psoriasis Vulgaris<br />

Introduction: Psoriasis is a chronic inflammatory disorder with polygenic<br />

predisposition and triggering environmental factors. It comprise several clinical<br />

expressions, typical lesions are chronic, recurring, scaly papules and plaques. For most<br />

affected patients psoriasis cause a deterioration of many aspects of daily life.<br />

Methods: We present a case of a psoriatic 56 year old man, affected since the age of<br />

20. During its long therapeutic history he has tried, for almost the totality, the current<br />

disposable drugs. He started with conventional treatments: radiation (Bade-PUVA),<br />

followed by ciclosporin, methotrexate, then it switched to the biologics such as raptiva,<br />

etanercep and adalimumab. In any case of therapy he had a short time remission<br />

followed by a resistant relapse of disease.<br />

Results: As final therapeutic option we give the patient the latest approved biological<br />

drug, Ustekinumab, with encouraging results: a first unambiguous PASI response was<br />

already seen within one month of therapy and almost complete remission in 3 months.<br />

The patient is still in therapy.<br />
