Background: Ro/SS-A antibodies are frequently detected in patients with lupus
erythematosus (LE). LE is a systemic autoimmune disease. However, some patients have
only cutaneous LE and others develop further internal organ involvement.
Objective: The prevalence of LE-specific and LE-non-specific cutaneous manifestations
and their relation to internal organ involvement in Ro/SS-A antibody positive patients
were determined.
Methods: 12 374 sera of patients were screened at our laboratory with an enzyme
immunoassay between 01/2000 and 12/2011. All patients with Ro/SS-A antibodies and
LE were enrolled and retrospectively analyzed.
Results: 215 patients were Ro/SS-A antibody positive and diagnosed with LE. Older
patients (> 50 years old) presenting with subacute cutaneous LE or chronic cutaneous
LE and negative ANA usually suffered only from skin involvement. In contrast, internal
organ involvement was observed in younger patients (< 50 years old) with subacute
cutaneous LE or chronic cutaneous LE presenting with fatigue and positive ANA. Young
female patients with acute cutaneous LE were a distinct group of Ro/SS-A positive
patients because almost 1/3 was diagnosed with kidney involvement. Logistic
regression analysis revealed that internal organ involvement was observed in LE
patients presenting with OR 2.4 for LE-non-specific cutaneous manifestations, OR 2.5
arthralgia, OR 2.1 for leucopenia, OR 2.8 for positive ANA and OR 2.4 for fatigue.
Limitations: It is a retrospective study from a single referral center specialized for
dermatological autoimmune diseases.
Conclusion: The particular cutaneous variant of LE as well as age at Ro/SS-A detection
predicts different risks for internal organ involvement in Ro/SS-A antibody positive LE
patients.
