Detection of Ro/SS-A antibodies in lupus erythematosus - what does it mean for<br />
the dermatologist?<br />

Background: Ro/SS-A antibodies are frequently detected in patients with lupus<br />
erythematosus (LE). LE is a systemic autoimmune disease. However, some patients have<br />
only cutaneous LE and others develop further internal organ involvement.<br />
Objective: The prevalence of LE-specific and LE-non-specific cutaneous manifestations<br />
and their relation to internal organ involvement in Ro/SS-A antibody positive patients<br />
were determined.<br />
Methods: 12 374 sera of patients were screened at our laboratory with an enzyme<br />
immunoassay between 01/2000 and 12/2011. All patients with Ro/SS-A antibodies and<br />
LE were enrolled and retrospectively analyzed.<br />
Results: 215 patients were Ro/SS-A antibody positive and diagnosed with LE. Older<br />
patients (> 50 years old) presenting with subacute cutaneous LE or chronic cutaneous<br />
LE and negative ANA usually suffered only from skin involvement. In contrast, internal<br />
organ involvement was observed in younger patients (< 50 years old) with subacute<br />
cutaneous LE or chronic cutaneous LE presenting with fatigue and positive ANA. Young<br />
female patients with acute cutaneous LE were a distinct group of Ro/SS-A positive<br />
patients because almost 1/3 was diagnosed with kidney involvement. Logistic<br />
regression analysis revealed that internal organ involvement was observed in LE<br />
patients presenting with OR <strong>2.</strong>4 for LE-non-specific cutaneous manifestations, OR <strong>2.</strong>5<br />
arthralgia, OR <strong>2.</strong>1 for leucopenia, OR <strong>2.</strong>8 for positive ANA and OR <strong>2.</strong>4 for fatigue.<br />
Limitations: It is a retrospective study from a single referral center specialized for<br />
dermatological autoimmune diseases.<br />
Conclusion: The particular cutaneous variant of LE as well as age at Ro/SS-A detection<br />
predicts different risks for internal organ involvement in Ro/SS-A antibody positive LE<br />
patients.<br />
