Because of the obvious inebriation symptoms after this procedure and the unpredictable release of ethanol out of the tumour into hepatic veins under forensic conditions therefore it is advisable that after PEI patients remain in the hospital under adequate monitoring in order to minimise the risk of iatrogenic ethanol intoxication. Keywords percutaneous ethanol injection – hepatocellular carcinoma – blood ethanol concentration – inebriation symptoms – forensic aspects Zusammenfassung Die perkutane Ethanolinstillation (PEI) unter sonographischer Kontrolle stellt eine nicht-chirurgische Möglichkeit zur Behandlung von Patienten mit einem hepatozellulären Karzinom dar. Die PEI kann insbesondere bei Patienten mit einem hepatozellulären Karzinom und einer fortgeschrittenen Zirrhose derzeit als eines der wenigen Behandlungsverfahren angesehen werden, mit dem eine Verbesserung der Lebensqualität sowie eine Verlängerung der Lebenszeit erzielt werden kann. Bei der Bestimmung der Blutalkoholkonzentration von 14 Patienten im Verlauf der PEI wurden Alkoholkonzentrationen im Blut von bis zu 1,62 ‰ erreicht, was zum Teil mit erheblichen Trunkenheitssymptomen im Rahmen der nachstationären Behandlung einherging. Da der Tumor in nicht vorhersagbarer Menge und Zeit Alkohol über die Lebervenen in den Blutkreislauf abgibt, zum Teil unmittelbar zu Beginn der Instillation bzw. auch prolongiert, können kinetische Betrachtungen in der Resorptions- und Eliminationsphase praktisch solange nicht angestellt werden, wie der Tumor Alkohol über die Lebervenen freisetzt. In diesem Punkt sind gegenüber der oralen Alkoholbelastung deutliche Unterschiede zur PEI immanent. Insofern ist eine vernünftige Abschätzung der Blutalkoholkonzentration im Verlauf der Instillationstherapie für den Kliniker, basierend auf der instillierten Menge Ethanols, wenngleich natürlich die Menge des instillierten Ethanols mit der maximalen Blutalkoholkonzentration statistisch korreliert, praktisch nicht möglich. Insbesondere ist somit in Vollnarkose mit einer iatrogenen Intoxikation zu rechnen. Infolgedessen ist unter forensischen Gesichtspunkten eine Beobachtung der Patienten im Anschluß an die perkutane Ethanolinstillationstherapie erforderlich, damit iatrogene Intoxikationserscheinungen rechtzeitg erkannt und behandelt werden können.
