Key factors for successful tooth transplantation 

Autotransplantation of developing teeth, preferably at 1/2 to 3/4 final root development, 

is a viable treatment option in growing patients with missing teeth. Teeth with developing 

roots have an ability to: establish a normal periodontal attachment, preserve the pulp 

vitality and adapt to the alveolar growth changes. Developing premolars are preferable 

donors because of their morphology and position in the dental arch. The protocol for 

transplantation of developing teeth was established at the University in Oslo more than 

4 decades ago and is successfully applied in Poland since 2000. 

The objectives of the course are to present: 

- orthodontic and surgical indications for the donor and recipient site selection including 

key elements in treatment planning, 

- surgical technique used in transplantation of developing premolars including clinical 

tips for a successful outcome, 

- follow-up protocol and signs of success and failure including possible solutions for 

healing complications, 

- orthodontic treatment related to tooth autotransplantation, 

- transplantations to maxillary anterior region including patients with concomitant tooth 

and bone loss and patients with clefts, 

- trans-alveolar transplantation of ectopically positioned teeth including key elements 

in treatment planning and surgical technique, 

- transplantation of wisdom teeth as an alternative to dental implants including key 

elements for satisfactory outcome, 

- potential for bone preservation and regeneration of transplanted teeth. 

The predictability of transplantation of developing teeth will be documented based on 

the studies performed at the University in Oslo and at the Warsaw Medical University. 

Various clinical examples will be used to illustrate the topics presented during the course 

and highlight possible clinical applications.

Management of patients with missing teeth in relation to the tooth transplantation 

and orthodontic space closure. 

The management of missing teeth in growing patients represents a major challenge for 

dental professionals. The replacement of teeth should not only provide satisfactory 

functional and esthetical outcome, but should also adapt to the inevitable growth 

changes, especially when in the anterior maxilla. Treatment planning for growing patients 

with missing teeth requires an comprehensive assessment of many aspects including 

type and number of missing teeth, space conditions, occlusion and profile, post 

treatment stability and the patient’s expectations. The management of anterior tooth 

loss is particularly challenging because of the esthetical demands in this region and the 

need for immediate replacement. Inter-disciplinary cooperation is essential for a 

successful outcome. 

The results of the short and long-term studies on auto transplantation of developing 

premolars and orthodontic space closure will be presented during the lecture, that will 

include predictability, functional and esthetical outcome. The critical aspects, which may 

contribute to sub-optimal outcome in the anterior maxilla, will be also highlighted. The 

patient’s opinion about the treatment process and of the treatment result will be 

compared with the professional assessment. Different clinical applications of tooth 

transplantations and orthodontic space closure will be shown.

Surgical considerations in autotransplantation of developing teeth. 

The lecture will provide information about planning and performing auto transplantation 

of developing teeth from the perspective of an oral surgeon. A detailed description of 

the surgical protocol and clinical tips for success, including factors, which influence the 

treatment outcome, will be given. Bone preservation and regeneration is an important 

issue in tooth transplantation and it will be discussed using various clinical examples. 

Developing teeth may in some cases serve as natural stimulators for bone growth and 

may also help in regeneration of alveolar process in patients with concomitant tooth 

and bone loss. The problem of ectopically positioned developing teeth, especially 

second premolars and canines, will be discussed regarding the possibility for transalveolar 

transplantation of such teeth. The lecture will also describe the follow-up 

protocol used in transplantation of developing teeth and its most important elements. 

Finally, the most common healing complication and their possible solutions will be 

discussed.

Kieferwachstum ist eine ganz entscheidende Komponente in der kieferorthopädischen 

Behandlung von Kindern. Deshalb ist es wichtig, dass wir wissen, wie das Kieferwachstum 

vorsichgeht. Besonders wichtig ist es zu verstehen was passiert wenn sich eine 

Malokklusion entwickelt. Es gibt Fälle, wo wir unbedingt früh anfangen müssen, und es 

gibt Fälle, wo wir unbedingt warten müssen. Vorhersage des Kieferwachstums ist hier 

sehr wichtig, aber ist es überhaupt möglich? Wachstums Vorhersage ist ganz entscheidend 

für die Behandlungsplanung. Hat das Kind ein gutes Wachstumsmuster können 

wir oft mit einer einfachen Behandlung zum Ziel kommen, ist das Wachstumsmuster 

aber schlecht wird es oft kompliziert. Wie das Wachstumsmuster die Behandlungsplanung 

beeinflusst wird genau besprochen. Der richtige Behandlungsplan ist eine wichtige 

Voraussetzung für ein schnelles und gutes Behandlungsergebnis und ein glückliches 

Kind.

Methoden zur Beurteilung und Beeinflussung des Längen- und Knochenwachstums 

Das Längenwachstum des menschlichen Organismus wird durch eine Vielzahl von 

Determinanten gesteuert und positiv beeinflusst, zum Beispiel durch genetische Faktoren 

(Zugehörigkeit zu Ethnien, Rassen, Familie, Verwandte, spezielle Gene), Umwelt, 

Ernährung, Stoffwechsel, und Hormone. Auch negative Faktoren sind bekannt, beispielsweise 

bestimmte angeborene oder erworbene Erkrankungen, Syndrome, Erkrankungen 

des Stoffwechsels oder des Endokrinums. Das Längenwachstum erfolgt 

unterschiedlich rasch und ist intrauterin und in der Säuglingsperiode besonders hoch, 

wird im Kindesalter etwas niedriger und dann im folgenden Pubertätswachstumsschub 

wieder hoch und ist mit dem Schluss der Epiphysenfugen (Wachstumsfugen) am Ende 

der Pubertät abgeschlossen. Danach können nur mehr die Akren wachsen. Während 

der Säuglings- und frühen Kleinkindsperiode spielt beim Wachstum die Ernährung eine 

besondere Rolle, danach Wachstums- und andere Hormone, und in der Pubertät die 

Sexual hormone. Zur Beurteilung von Größe und Wachstumsgeschwindigkeit werden 

meist sogenannte Perzentilenkurven herangezogen. Von großer Bedeutung ist auch die 

Bestimmung des sogenannten Knochenalters (das nicht mit dem chronologischen Alter 

ident sein muss) und eine Prognose der Endgröße erlaubt. Finden sich Abweichungen 

zur Norm ist eine Diagnostik erforderlich, um Störungen möglichst frühzeitig zu erfassen 

und eventuelle therapeutische Maßnahmen einzuleiten. Diese können entweder kausal 

oder symptomatisch sein, und das Längenwachstum positiv (bei Kleinwuchs) oder auch 

negativ (bei Großwuchs) zu beeinflussen versuchen.

WIRELESS – Invisalign ® eine ernstzunehmende Alternative zur festsitzenden 

Therapie? 

Die Einsatzmöglichkeiten von Invisalign® Schienentherapie haben sich im Laufe der 

Jahre, aufgrund vieler Innovationen, enorm vergrößert. Die steigende Nachfrage nach 

unsichtbaren Zahnspangen, hat auch das therapeutische Spektrum der Kieferorthopäden 

dementsprechend verändert. Anhand zahlreicher Fallbeispiele, unterstützt durch 

computeranimierte Clin Checks, bietet der Vortrag einen Querschnitt durch die umfassenden 

Einsatzmöglichkeiten von Invisalign® Schienentherapie. Beginnend mit der

DDr. Bärbel Reistenhofer studierte Medizin an der Universität 

Graz und Wien. Ihr Studium der Zahnmedizin absolvierte sie

Während der siebenjährigen Mitarbeit an der Abteilung für 

Kieferorthopädie der Universitätszahnklinik Wien bei Prof.

spezialisiert. Sie ist Active Member der "European Lingual 

Orthodontic Society" und der "World Lingual Orthodontic

Growth of the face: Can it be predicted? 

Evaluating dental and facial change as a result of orthodontic treatment in a growing 

child is difficult. Traditionally, dento-facial growth has been evaluated using model and 

cephalometric analyses. Surprisingly, less attention has been given to soft tissue 

evaluations seeing this is the most visible part of the face and shows greater change 

than the underlying skeletal framework particularly from the age of 11-20. Threedimensional 

assessment of growth and treatment changes enables quantification in 

terms of surface topography/area and volume. Growth becomes more complex as facial 

anatomical landmarks don’t exist in the same sense as projected landmarks. The aim 

of this study is to demonstrate three-dimensional changes/growth of the face in a 

sample of Welsh and Finnish children followed from the age of 11 to 17 years of age. 

The growth of the individuals will be presented on a standardized framework with a 

common origin highlighting different growth patterns. The growth patterns will be 

discussed in the context of different facial characteristics derived from a large population 

(4747 15 year olds). The use of growth prediction based on different facial characteristics 

at the age of 12 will be discussed. In addition the influence of medical condition on 

face shape will be highlighted. 

Born in Liverpool, gained dental degree in Sheffield (1979) 

followed by an orthodontic specialist training qualification in 

Cardiff (1984). Lecturer and Senior Lecturer in the University 

of Manchester (1985 – 1993) and attained doctorate in 

relation to quantifying orthodontic treatment need and 

outcome (standards) in the General Dental Services in 

England and Wales. Moved back to Cardiff in 1993 and 

currently Head of Applied Clinical Research and Public 

Health. Best known for developing and implementing the use 

of the PAR, IOTN and ICON indices and assessing the costeffectiveness 

of orthodontic treatment. Recently studies 

have focused on three-dimensional facial growth and growth 

prediction (12-17 yr olds) in different population groups and 

the influence of environment and genetics on face shape. 

Interests also extend to exploring the use of biomechanical 

modelling to plan and simulate facial surgery. Recent 

research grants exceed €3 million supported by over 200 

publications.

Treatment concepts for Class II malocclusions : how to take advantage of the 

growth and the dento-skeletal features 

Class II and Class III are challenging malocclusions to treat, especially when the patient 

is a growing kid.If optimal final result is dubious, the clinician is often uncertain about 

the priority of each treatment objective: esthetics, skeletal balance, occlusion, and so 

forth. 

The future growth and the efficacy of the available treatments are not always one 

hundred percent predictable. The information coming from the research is also often 

quite distant from the everyday clinical work and findings. 

This paper, divided in two parts (the first one about Class II and the second about Class 

III), will try to cover the clinical aspects of the early treatment and management of Class 

II and Class III malocclusions. 

In Class II malocclusions, different situations will be presented when to distalize upper 

molars or when to protrude the mandible to try to stimulate its growth or when just 

expand the upper arch. 

In Class III malocclusion, situations when to expand the upper arch or when to apply 

postero-anterior orthopedic forces to try to stimulate maxillary growth will be 

discussed. 

Some biomechanical aspects about the following finalization in permanent dentition will 

be covered, as well. In the era of evidence based orthodontics, these two papers, even 

if based on the information coming from the most recent researches, will be mostly 

clinical. 

Dr. Raffaele Spena received his degree in Odontoiatria e 

Protesi Dentaria at the II Facoltà di Medicina e Chirurgia of 

Napoli in 1984. In 1988, he received his Certificate in 

Orthodontics at the Dental School of the University of 

Pennsylvania, Philadelphia. 

In 2003, he received the degree of “Specialty in Orthodontics” 

at the University of Ferrara. 

He has been and still is involved in several research projects, 

but he has mainly co-worked for more than 15 years with 

Dr. Norman Cetlin and Arne Ten Hoeve with whom Dr Spena 

has given courses in Italy, Europe and USA. 

He has been speaker at both Italian (SIDO,SITEBI,ANDI, 

Italian Accademy of Orthodontics) and international meetings 

(AAO, EOS, Israel Ortho Society, Israeli Periodontal & Osteointegration Society, 

Sociedad Espanola de Ortodoncia, Nederlandse Vereniging Voor Orthodontische 

Studie, Hungarian Ortho Society, etc. ). 

In July 1998, he published an atlas about “Il trattamento senza estrazioni” 

(Nonextraction technique). 

In 2000, he published with the drs. R.L.Vanarsdall and N.M. Cetlin a chapter 

“Nonextraction Treatment” in the textbook “Orthodontics – Current principles 

and techniques” by T.M.Graber e R.M.Vanarsdall. A updated chapter, written with 

the same co-authors, has been included in the new editions (2005 and 2011) of 

the same book edited by T.M.Graber, R.M.Vanarsdall e K.W.L.Vig. 

In 2002 he published “Nonextraction Treatment : an atlas on Cetlin’s mechanics” 

edited by Fenske Media Corporation.

Dr. Spena has been Adjunct Clinical Professor at the Orthodontic Department of 

the Dental School of the University of Pennsylvania since 1988 where he has 

given courses about nonextraction therapy and cephalometric analysis. 

He has also been Adjunct Clinical Professor at the University of Parma from 1996 

to 2000. At the moment, he is Clinical Professor at the University of Ferrara. 

He is a member of the American Association of Orthodontists, Società Italiana 

di Ortodonzia, Societa’ Italiana di Odontoiatria Pediatrica, World Federation of 

Orthodontists and European Orthodontic Society. 

Dr. Spena has been President of the Italian Accademy of Orthodontics for the 

year 2010 and has been Scientific Secretary of the same society in the years 

2008-2009. 

He is an active member of the Angle Society of Europe. 

In 2001, he has received the Certificate of Excellence in Orthodontics in Italy and, 

in 2008, the E.B.O. (European Board of Orthodontics) in Lisbona. 

He practices orthodontics in Napoli.

The Sato Concept Revisited: Growth and Treatment 

Nach Sato spielt neben der Vertikaldimension die Neigung der Okklusionsebene eine 

zentrale Rolle für das Verzahnungsmuster und damit auch für die Entstehung einer Malokklusion. 

Seine Empfehlung lautet, mittels Kieferorthopädie die Okklusionsebenenneigung 

gegebenenfalls zu verändern, um sowohl eine Klasse II- oder Klasse 

III-Malokklusion als auch Laterognathien korrekt zu behandeln. 

Der Vortrag erläutert theoretische und praktische Grundlagen, erörtert die bestehende

Erfolg oder Misserfolg in der Klasse II-Behandlung – welche Faktoren zählen? 

Im Verlauf der Vorlesungseinheiten werden anhand der verfügbaren Daten in der 

internationalen Literatur und eigener bisher unveröffentlichter Ergebnisse erfolg- bzw. 

misserfolgbeeinflussende Faktoren für eine kieferorthopädische Behandlung analysiert. 

Der Schwerpunkt dieser Analyse liegt bei der Klasse II-Behandlung. Ziel dieser Analyse 

ist es, dem praktisch tätigen Kieferorthopäden Richtlinien für die tägliche Praxis 

mitzugeben. 

Daneben wird das persönliche Klasse II-Therapiekonzept mit herausnehmbaren und 

festsitzenden Klasse II-Apparaturen vorgestellt. Dabei wird unter anderem auf folgende 

Faktoren eingegangen: (1) Angle Klasse II:1 und II:2 Dysgnathien, (2) hyperdivergente 

Patienten, (3) optimaler Behandlungszeitpunkt, (4) Behandlung von jugendlichen und 

erwachsenen Patienten, (4) Stabilität, (5) Rezidiv.

This innovative course is directed at specialist orthodontists. We will, uniquely, combine 

clinical lectures with a scientific critique of the evidence underpinning our treatment. 

There will be ample time for informed discussion. This course is an extension of our 

highly rated combined keynote presentations at Conferences in Sweden, Ireland, UK 

and the post congress course at the AAO meeting in 2011.

practice. He is one of two UK members of the Edward H 

Angle Society of Orthodontics North Atlantic Component 

and past editor of the Journal of Orthodontics 


Jonathan has been a Consultant Orthodontist at Chesterfield 

Royal Hospital since 1993. He was Postgraduate Course 

Director at Manchester University from 1995-2002. 

He is has been an M.Orth Examiner since 1997 for both the 

Royal College of Surgeons of England and Edinburgh. He 

was an ISFE Examiner from 2001-2007. 

He is a full member of the Angle Society of Europe, and one 

of only two British orthodontists with European Board 

Certification in Orthodontics.
