Deep Hypothermia in Hot Summer and the Bernese Hypothermia Algorithm

We present the case of a 47-year old mountaineer who suffered from deep hypothermia after a fall into
a crevasse in summer 2010. On-site he was in cardiac arrest. Chest compressions were started,
intubation was performed, and the patient was transported under continuing CPR during the flight. In
the University Hospital Bern he was directly brought to the operating theater. Deep hypothermia with a
core temperature of 21.4°C was confirmed, and based on the new Bernese Hypothermia Algorithm
active rewarming by ECC was started 27 minutes after the patient’s arrival. Due to massive
rhabdomyolysis the patient was on hemodialysis postoperatively, but the overall and especially the
neurological course was very favorable.
