placeret sundhedsprofessionel. Derudover mangler der enighed<br />

om opgaverne for en forløbskoordinator, så funktionen opfattes<br />

ens af sundhedsprofessionelle og patienter.<br />

Vi mener, at forløbskoordinatoren er centralt placeret i forhold<br />

til at sørge for, at relevant information videregives til en anden<br />

sundhedsperson eller en ny forløbskoordinator, som tager ansvar<br />

for det videre forløb. Den praktiserende læge bør f.eks. i højere<br />

grad end nu informeres om det planlagte forløb og om psykosociale<br />

problematikker. Vi tror, at det kan være med til at styrke<br />

patientens oplevelse af et sammenhængende sundhedsvæsen og<br />

derved give en følelse af tryghed. Uanset hvem informationen<br />

videregives til, kan en forløbskoordinator sikre, at informationsstrømmen<br />

løber foran patienten, og på den måde give andre<br />

implicerede mulighed for at være forberedt inden mødet med<br />

patienten. På den måde sikrer vi optimale patientforløb før, under<br />

og efter indlæggelse.<br />

is the patient’s safety net. Sygeplejersken 2013;(3):64-8.<br />

This article describes two nurses’ experience as case managers in<br />

a research study, the purpose of which was to analyse the effect<br />

that having case managers made in the treatment of colorectal<br />

cancer patients.<br />

The study’s case managers are experienced professionals who<br />

were not only given time to monitor the course of treatment and<br />

respond to calls from patients, but were also proactive and involved<br />

in how events proceeded.<br />

The study found that patients and families appreciated having<br />

a familiar healthcare professional available for them when they<br />

were in doubt, in need of support or had questions about the<br />

treatment process. The nurses experienced the role of case manager<br />

as meaningful, as they acted as the patient’s safety net and<br />

created consistency throughout the complex course of events.<br />

It was found that experienced nurses having knowledge about<br />

the organization of the health system and its opportunities for<br />

collaboration make them prime candidates to act as case managers.<br />
